We ask all new patients to provide 2 forms of identification upon registering.
A combination of the following will be accepted as identification We prefer that one
item is photo ID and one document contains your address
Examples of suitable ID :
local authority rent card
paid utility bills
bank/building society cards/statements
National Insurance number card
letter from Benefits Agency/benefit book/signing on card
papers from the home office
